Welcome to Timberwood Coaching & Counseling! We’re committed to providing you with personalized support on your journey to well-being. To get started, please complete the following intake forms. These forms will help us understand your unique needs and tailor our services to best assist you. If you have any questions or require assistance, please don’t hesitate to contact our office.

Client Intake Forms

In preparation for your first appointment at Timberwood Coaching & Counseling, we kindly ask that you complete the following forms and email to support@timberwoodscounseling.com:

  1. Timberwood Informed Consent & Confidentiality Forms 1 & 2: These forms outline our policies regarding confidentiality, privacy, and your rights as a client.

  2. Timberwood New Client Information Form: This form helps us gather important background information to better understand your needs and tailor our services accordingly.

Once completed, you can easily submit them all at once by emailing them to support@timberwoodscounseling.com. We recommend completing these forms at least 24 hours before your initial appointment to allow us to process the information and ensure a smooth start to our work together. BILLING | FEES:
  • INITIAL EVALUATION: (75 minutes) $160
  • INDIVIDUAL COACHING/COUNSELING SESSION: (50 minutes) $110

PAYMENT(s)

Payment is required at the time of service. The practice accepts cash or Visa/MasterCard/Discover/AmEx.

CANCELATIONS

You will be billed in full for missed appointments and those canceled without 24 business hours’ notice. Remembering your appointment is your responsibility. Your credit card on file will be charged.
